Looks to me like the center broke broke out of the adapter and the threaded part with the tightening tabs remained on the faucet

Swivel Adapters connect 1/2" PEX tubing to male NPSM faucet connections. Just cut old off and slip this connector in along with a cinch ring. You can buy pex cinch pliers and cinch rings fairly...
