I saw you said you tried to twist and pull but rust may make it harder than usual to pull off. Try again, with gusto.

Some vent covers had a spring tension clip to hole the top on. It might simply pull up and off. Since has so much rust it might be harder than normal to pull off. Maybe try to twist the cap to see...
