The actual hitch head in on correct. No worries there.
I do not understand what you mean the 2 lugs look a bit crooked. What are you calling a lug? Do you mean the end of the bolts with the nuts on them holding the hitch head to the hitch shank? If so they look normal to me from this pic.
Now to the WD bars, Need more info on how close to the ground they are and how the hitch is adjusted. We need to help you get the WD bars to work correct with the hitch. When you get a chance, put them on, tighten them up and take some side pictures of the truck, hitch and camper. Measure the ground to the bottom of the WD bar. That distance will tell us a lot.
We also need the following measurements: Truck and TT to be on hard surface level ground. Also ideally loaded with weight to go camping.
Before hitching up.
1. Measure the truck fender wells both front and rear distance.
2. Then hitch up, put the bars on and tighten the chains as the guy told you.
3. Then go measure the truck fenders again.
I mean measure the fenders like this. Ground to inside of fender and through the center of the axle.
Once hitched, the WD bars, on level ground, tell us is the camper nose up, nose down, of level? You can put a level on the siding like this or the bottom of the camper frame.
Then tell us how many inches the camper ball coupler out on the end is up hill, down hill or dead on level. Measure the best you can.
Tell us the numbers teh ball coupler is from camper being level
Your truck needs the WD bars to work correct. The back is dropping when you put the TT tongue on the ball even if you cannot notice it right off. You can even measure the rear and front fenders by letting the weight hang on the ball with no WD bars on. The rear is going down and the front is going up from the truck unhitched. We need to see what you are up against on the ground clearance o the WD bars to help guide you on who to get out of the problem.
